The post holder will conduct comprehensive Continuing Healthcare and Funded Nurse Care assessments for adults with complex health needs, ensuring timely assessment, review, and monitoring.
- Follow the National Framework for NHS Continuing Healthcare, including organising and coordinating MDT eligibility recommendations for ICB verification.
- Support delivery and implementation of Personal Health Budgets (PHBs) and DOLS/LPS for the assigned caseload.
- Ensure the clinical team maintains data systems per standard operating procedures.
- Handle daily CHC Service inquiries and unexpected case management issues.
QUALIFICATIONS & JOB REQUIREMENTS:
- 6 months of Healthcare experience as a Lead Complex Case Manager
- Right to work in the UK
- References covering the last 3 years of clinical employment
- NMC Registration
- Overseas Police Check, if any
- Indemnity Insurance
- Up-to-date CV
